Dental Savings Plan Enrollment FormTerms and Conditions
- All family members must live in the same household.
- This is a dental discount plan and not dental insurance and cannot be combined with any other insurance plan.
- Membership Dues are non-refundable and must be paid in full before enrollment begins.
- If payment is required, it is due in full at the time of service.
- The effective date of the plan is the day you enroll and is good for 12 consecutive months.
- Cleanings must be completed before the end of the coverage date.
- Membership is limited to Broad Smiles Pediatric Dentistry & Orthodontics, if you must be referred to a specialist, discounts will not apply at that location.
- Membership fees or percentage discount is subject to change on a yearly basis.
- Procedure fees are subject to change at the office’s discretion.
